Patients who are neutropenic have a reduced ability to fight infection and are at increased risk of developing neutropenic sepsis. Nurses need to be able to recognise the signs and symptoms of neutropenic sepsis to ensure early diagnosis and treatment. 2019-02-22 · Sepsis and septic shock are major causes of mortality during chemotherapy-induced neutropenia for malignancies requiring urgent treatment. Thus, awareness of the presenting characteristics and prompt management is most important. Improved management of sepsis during neutropenia may reduce the mortality of cancer therapies.

We agree with using this definition of fever in neutropenic patients. Febrile neutropenia is defined as a one-time oral temperature of greater than 38.3°C (approximately 100.9°F) or a sustained temperature of greater than 38°C (100.4°F) for ≥ 1 hour in a patient who has an absolute neutrophil count of less than 500 cells/μL or an absolute neutrophil count expected to decrease to less than 500 cells/μL within a 48-hour period. 2 Typically, the onset of neutropenia occurs approximately 1 week after delivery of cytotoxic chemotherapy.

7 8 Information and support for patients and carers Neutropenic sepsis is a life-threatening reaction to an infection, which can happen in patients with neutropenia (a low level of neutrophils in the blood). Neutrophils are a type of white blood cell that work as part of the immune system to fight infection.

Neutropenia that occurs with common childhood viral diseases develops during the first 1 to 2 days of illness and may persist for 3 to 8 days. Diagnosis of neutropenic sepsis (as per NICE guidelines Sept 2012) is patients having anticancer treatment whose neutrophil count is 0.5 x 109 /L or lower and who have either a temperature higher than 38 0 C OR other signs or symptoms consistent with clinically significant sepsis. Although neutropenic sepsis is a common, costly and not infrequently fatal complication in haematology and oncology, it lacks clear, national, evidence‐based guidelines informing urgency of antibiotic administration. Accordingly, for stable patients without sepsis, pneumonia, mucositis, or evidence of line infection, there is no benefit to empirically adding vancomycin to the initial empiric regimen used for management of neutropenic fever. 19,20 Several clinical practice guidelines exist to help with management of febrile neutropenia. 2,7,21 Neutropenic sepsis is a potentially life-threatening complication of neutropenia (low neutrophil count). It is defined as a temperature of greater than 38°C or any symptoms and/or signs of sepsis, in a person with an absolute neutrophil count of 0.5 x 109/L or lower.
